Medical *Care plan coordination in conjunction with nursing, treating doctors,physiotherapy and other allied services. *Consolidation and development of multi-service care plan to allinvolved services *Communication of care plan goals to all involved departments *Monitoring of delivered care to ensure evidence based practice andpatient centered focus
Financial *Closely work with financial, pre-cert and third party payer to ensureaccuracy of information and efficient communication with patient. *Liaise with patient’s family to explain financing in conjunction withFinance and Pre cert departments. *Support frontline financial services staff to communicate with medicalteam, patient and/or third party payer to improve the efficiency,accuracy and outcome. *Coordinate and monitor medical service referral visits outside BJU.
Educational *Consolidation of care plan details into an easily understood format forpatient and family consultation.
Psychosocial *Identification of psychosocial needs. *Referral to chaplaincy, psychology services etc. as needed. *Patient advocacy.
Research *Collection and analysis of data as requested by hospital departments. *Identification and monitoring of quality indicators. *Development of applicable policies and procedures. *Participation in BJU administrative/clinical planning – meetingattendance.
Qualifications: • Bachelor’s degree in Social work, Nursing or allied medical field • Previous experience in Case management desired • Previous hospital experience a must • Bilingual Chinese English a must – additional language competence aplus
