DUTIES AND RESPONSIBILITIES: Coordination of patient care 1. Be single point of contact and manage all aspects of new and existingpatient queries 2. Identify and develop relationships with personnel in departmentsinvolved in the care of patients. 3. Facilitate new patients medical visit by reviewing records andapplications in advance 4. Coordinate services need to be provided include but not limited toarrange resources, set schedule, transportation and escort. 5. Master services and be able to introduce and offer the correctservices to patients and their families 6. Assess treatment needs, develop, monitor, evaluate treatment plan andprogress for cancer patients according to physician and nursing’streatment plan 7. Monitor cases by checking patients’ attendance; evaluating patients’ experience. 8. Maintain patients’ records by reviewing case notes; logging eventsand progress 9. Ensure appropriate health information and education about theirdiagnosis and treatment plan. 10. Engage patients and their families to be part of the treatmentprocess to the degree their desire 11. Develop and responsible for patient support groups and patienteducational workshops
Communication between patients and other healthcare professionals 1. Develop effective working relations and cooperate with otherhealthcare professionals and patients/their families throughout theentire case management process to ensure the achievement of quality careand best possible outcomes for patients 2. Participate and communicate patients’ progress in interdisciplinary meetings
Support patients financial decision making process 1. Assess the needs of patients to enable the most appropriate provisionof services in a timely and cost effective manner 2. Provide financial counselling for all patients; initiate and followup GOP requests 3. Enhance and achieve new patient treatment conversion and existingpatients’ retention target set by service units 4. Develop plans to optimize and improve conversion ratios
