岗位职责: Primary Duties 主要职责： • Reviews and interprets prescription orders and verifies accuracy andcompleteness of patient labeling. • 评价、审核处方，核对标签上患者信息的准确及完整性 • Evaluates and resolves real or potential prescription and medicationrelated problems. • 评估并解决处方中存在或潜在的问题 • Provides patient consultation on medications • 为患者及医护人员提供药物的信息咨询 • Establish a pharmaceutical care practice in conjunction withphysicians and other care providers. • 与医生及其他医疗工作者一起建立药学实践服务 • Reviews inpatient orders and screens them for possible duplications, interactions. • 评价、审核病房医嘱，及时发现潜在的禁忌证、相互作用 • 指导病房(区)护士请领、使用与药品管理 • Dispense medication to hospitalized patients • 为住院及出院患者分发药品 • Follow and understands all pharmacy policies and relevant hospital policies. • 理解并遵循药品管理相关的规章制度
Other Duties Performed 其他职责： • Ordering inventory and doing periodic inventory check. • 定购药品并定期盘点库存 • Performing periodic narcotic inventory. • 定期盘点麻醉药品的库存 • 检查药品的有效期 • 每月与药品供应商结算发票 • Take part in core team pharmacist duties. • 参与核心药师团队的轮转 • Participate in holiday shifts if needed. • 参加夜班与周末值班，若需要可进行电话值班 • Provide assistance and training to residents and pharmacy technicians. • 为药师培训生和药师助理提供帮助和培训 • Take part in clinical or other projects as assigned by supervisor ormanager. • 开展抗菌药物临床应用监测，实施处方点评与超常预警，促进药物合理使用 • 开展药品质量监测，药品严重不良反应和药品损害的收集、整理、报告等工作 掌握与临床用药相关的药物信息，提供用药信息与药学咨询服务，向公众宣传合理用药知识 • Plus other tasks as assigned by senior pharmacist, pharmacysupervisors and pharmacy managers • 包括资深药剂师，药房主管和药剂科主任指派的其他任务 任职要求: Qualifications学历和专业技能要求:
• Bachelor’s degree in pharmacy or equivalent • 正规药学相关专业学校毕业，本科及以上学历 • 3 to 5 years pharmacy work experience or < 1 year pharmacy workexperience in joint venture or international medical institution • 3-5年药学相关经验或者1年在合资或外资医疗机构的药学工作经验 • Licensed pharmacist or pharmacist title is required • 有药学专业技术职称 Skills知识与能力的要求:
• Be able to perform the pharmacy duties independently能独立完成工作职责 • Good communication skills in English and Chinese. 良好的中英文沟通能力 Knowledge:
• Knowledge of government laws pertaining to drugs, narcotics andpharmacy practice. • 熟悉政府相关机构对药品、麻醉药品以及药学实践的相关法律法规 • Knowledge of commonly used drugs and pharmacy inventory • 常用药品及药品管理的知识
