岗位职责: Job Description: Provide full-cycle health management services for United Family Prime members. Through proactive care, precise follow-up, and resource coordination, create a high-quality service experience to help members achieve their health goals. 职位描述：为和睦家Prime会员提供全周期健康管理服务，通过主动关怀、精准随访与资源协调，打造高品质服务体验，助力会员健康目标达成 Main Responsibilities: 主要工作内容： Establish and maintain exclusive service relationships with members.建立并维护会员专属服务关系; Proactively plan and execute health management actions such as appointment scheduling, post-consultation follow-up, re-examination reminders, report reminders, and benefit planning.主动规划并执行预约、诊后随访、复诊提醒、报告提醒、权益规划等健康管理动作; Collaborate with PCP (Primary Care Physician) family doctors to track and intervene in personalized health plans.协同PCP家庭医生完成个性化健康方案的跟踪干预; Handle member inquiries and requests in a closed-loop manner, responding on the same day and following through to resolution.闭环处理会员咨询与诉求，当日响应并跟进至解决; Operate digital health management tools, maintaining member health records and ensuring data quality.运营数字化健康管理工具，维护会员健康档案与数据质量; Support marketing activities, benefit promotions, and coordination with hospital departments.配合市场活动、权益推广及院区协同工作. 任职资格: Qualifications: 任职要求： College diploma or above in a medical-related field (Nursing/Clinical Medicine/Public Health).大专及以上医学相关专业（护理/临床医学/公共卫生); Strong communication and empathy skills, able to explain medical knowledge in an easy-to-understand manner.良好的沟通共情能力，医学知识表达通俗易懂; Proficient in using multi-modal communication tools with basic digital service capabilities.熟练使用多模态沟通工具，具备基础数字化服务能力; Strong proactive service mindset, adept at anticipating customer needs.主动服务意识强，善于预判客户需求. Preferred Qualifications: 加分项： Holds a Registered Nurse Practicing Certificate with over 3 years of clinical nursing or health management experience.持有注册护士执业证书，3年以上临床护理或健康管理经验; Strong English/Cantonese listening, speaking, reading, and writing skills.英语/粤语听说读写能力佳者; BLS certification, Health Manager (CHC), or chronic disease management-related qualifications.BLS认证、健康管理师（CHC）或慢病管理相关资质.
