• Be single point of contact and manage all aspects of new andexisting patient queries
• Identify and develop relationships with personnel in departmentsinvolved in the care of patients.
• Facilitate new patients medical visit by reviewing records andapplications in advance
• Coordinate services need to be provided include but not limited toarrange resources, set schedule, transportation and escort.
• Master JICC services and be able to introduce and offer thecorrect services to patients and their families
• Assess treatment needs, develop, monitor, evaluate treatment planand progress for cancer patients according to physician and nursing’streatment plan
• Monitor cases by checking patients’ attendance; evaluatingpatients’ experience.
• Maintain patients’ records by reviewing case notes; logging eventsand progress
• Ensure appropriate health information and education about theirdiagnosis and treatment plan.
• Engage patients and their families to be part of the treatmentprocess to the degree their desire
• Coordinate discharge plan and post-discharge requirements;educating and training family members
• Develop and responsible for patient support groups and patienteducational workshops
2. Communication between patients and other healthcare professionals
• Develop effective working relations and cooperate with otherhealthcare professionals and patients/their families throughout theentire case management process to ensure the achievement of quality careand best possible outcomes for patients
• Participate and communicate patients’ progress ininterdisciplinary meetings
3. Support patients financial decision making process
• Assess the needs of patients and their families to enable the mostappropriate provision of services in a timely and cost effective manner
• Provide financial counselling for all patients; initiate andfollow up GOP requests
• Enhance and achieve new patient treatment conversion and existingpatients’ retention target set by cancerhub
• Develop plans to optimize and improve conversion ratios

• Support marketing by completing surveys
• Promote JICC and hospital public relations
• Adheres to professional standards, policies and procedures,quality/safety standards; updates job knowledge by continuous learning;exploring opportunities to provide value added services to patients
• Demonstrate profession’s clinical standards and Code of Ethics
• Ability to navigate systems with patients/families. Shareknowledge of community agencies and resources.
